Echocardiography (Echo)

An echocardiogram uses high-frequency sound waves to produce real-time images of the heart's structure and function — allowing physicians to evaluate chambers, valves, wall motion, and blood flow without radiation or invasive procedures.

Why Is an Echocardiogram Ordered?

  • To evaluate heart function and measure ejection fraction in patients with suspected or known heart failure
  • To assess heart valve disease — stenosis, regurgitation, or prolapse — and determine severity
  • To detect structural abnormalities such as cardiomyopathy, septal defects, or congenital heart disease
  • To evaluate pericardial effusion or cardiac tamponade following trauma or infection
  • To monitor response to cardiac medications or after cardiac procedures and surgeries
  • To screen high-risk patients before major surgery or chemotherapy

Possible Causes & Clinical Indications

  • Chest pain or pressure — rule out ischemic cardiomyopathy or wall motion abnormalities
  • Shortness of breath / dyspnea — evaluate systolic or diastolic dysfunction, valvular disease
  • Heart murmur — characterize valve pathology (aortic stenosis, mitral regurgitation, MVP)
  • Atrial fibrillation / arrhythmias — assess left atrial size, thrombus, structural cause
  • Hypertension — evaluate left ventricular hypertrophy and diastolic function
  • Syncope or near-syncope — rule out outflow obstruction or severe valvular disease
  • Stroke / TIA — identify cardioembolic source such as thrombus or PFO
  • Edema / volume overload — assess cardiac vs. non-cardiac etiology

Why Is Vascular Ultrasound Ordered?

  • Arterial Upper Extremity — evaluates blood flow in the arm arteries to detect stenosis, occlusion, subclavian steal syndrome, or Raynaud's disease causing arm pain, coldness, or weakness
  • Arterial Lower Extremity — assesses leg artery flow to diagnose peripheral arterial disease (PAD), claudication, and critical limb ischemia; includes ankle-brachial index (ABI) measurement
  • Venous Insufficiency — maps venous reflux, incompetent valves, and chronic venous disease in upper and lower extremity veins, including DVT detection and varicose vein evaluation
  • Carotid Ultrasound — images the carotid arteries for atherosclerotic plaque, stenosis, and intima-media thickness as stroke and TIA risk stratification
  • Abdominal Aorta — evaluates the abdominal aorta for aneurysm (AAA), renal arteries for stenosis, and mesenteric vessels for occlusive disease

Possible Causes & Clinical Indications

  • Arm pain, coldness, or weakness — arterial upper extremity study for subclavian/brachial stenosis, occlusion, or Raynaud's syndrome
  • Claudication or leg pain with walking — arterial lower extremity for PAD, critical limb ischemia; ABI < 0.9 confirms disease
  • Leg swelling, heaviness, or varicose veins — venous insufficiency study for reflux, incompetent valves, chronic venous hypertension, or DVT
  • TIA, stroke, or carotid bruit — carotid ultrasound to detect plaque, stenosis >50%, and assess stroke risk
  • Pulsatile abdominal mass or AAA screening — abdominal aorta ultrasound for aortic aneurysm in patients 65+ or with family history
  • Resistant hypertension or declining renal function — abdominal aorta / renal artery duplex for fibromuscular dysplasia or renal artery stenosis
  • Non-healing wounds or diabetic foot ulcers — lower extremity arterial & venous study to guide wound care and revascularization decisions
  • Pre-operative vascular mapping — arterial and venous assessment before bypass, fistula creation, or vein graft harvest
